Vitiligo is a long-term condition that causes pale, white patches (loss of skin colour) to develop on the skin – in severe cases these cover the whole body. Vitiligo affects one in every hundred people and can strike anyone at any time. The condition is not life-threatening or contagious,but often leads to depression and isolation – especially amongst children.

What is Vitiligo?
Vitiligo is an auto immune disease, in which the person’s own immune system recognizes self-skin cells that produce melanin (melanocytes) as foreign and destroys them, resulting in white patches on the skin. Melanin is the protein that gives color to the skin, hair and eyes.

Some of the symptoms of Vitiligo are:
- Prominent white patches on different body parts or localized area such as face, hands or feet.
- Increase in the area of the white patches with time.
- Color change on inner side of the mouth or nose.
- Premature whitening of the hair on head, eyelashes, eyebrows, or face.

What are the different types of Vitiligo?
Vitiligo can be characterized into two types, based on the distribution of the white patches over the body.
- Localized: In this kind the white patches are confined to a particular area of the body. This is further classified into three types; focal, segmental and mucosal.
- Focal: This type is characterized by one or more areas of pigment loss in a confined area.
- Segmental: This type is characterized by the patches on either side of the body.
- Mucosal: In this type the mucous membranes are affected, depigmentation can be seen inside the mouth or on the genitals.
- Generalized: In this kind the white patches are not confined to a particular area of the body. This is further classified into three types: Acrofacial, Vulgaris, Universal.
- Acrofacial: Depigmentation can be seen on the face, head, hands and feet.
- Vulgaris: This is characterized by scattered patches that are widely distributed.
- Universal: In this type, depigmentation occurs on the whole body
What are the causes of Vitiligo?
The main cause of this skin condition is autoimmune disorder, in which the body’s own immune system attacks its skin’s melanocytes or pigment producing cells. This autoimmune disorder can be triggered by emotional, physical or mental stress. Some more factors which can indirectly link to this skin condition are family history of auto immune disorders, like rheumatoid arthritis, lupus or thyroid disease or family history of vitiligo.
How is Vitiligo diagnosed?
If white patches are observed on the body, one should get it examined by a Dermatologist. The doctor will do the following things to diagnose this skin condition:
- Thorough examination of the white patches, will look for patch distribution in other body parts.
- The spots/ white patches will be checked by illuminating that area of skin, using Ultraviolet light. Vitiligo patches turn fluorescent under the UV lamp’s light.
- Skin Biopsy is done by removing a small portion of patchy skin tissue to check for melanocytes in the skin. Vitiligo is confirmed if no pigment cells are observed.
- Blood test is done to find out any auto immune disorder such as thyroid dysfunction, as vitiligo is associated with auto immune disorder. Also, complete blood count test (CBC test) or an antinuclear antibody test (ANA test) is performed.
- Eye test will be done to check for inflammation in the coloured part of the eyes (uveitis).
Are all white patches Vitiligo?
No, there are a few more skin related disorders which cause skin pigmentation. These are
- Albinism: Albinism occurs because of a defect in the gene that affects melanin production. This results in either little or no color to skin, eyes or hair.
- Hypopigmentation: Hypopigmentation mostly develops as a result of injury or trauma like skin burns, blisters or infections that damages the skin and leads to its discoloration.
- Pityriasis Alba: Pityriasis Alba is common in children or young adults, symptoms being dry, fine scaled pale patches on the face.
